Abstract：Traditional chronic disease management methods are inefficientand dificult to metthe medical service requirements of patients.Combined with theactual situation of hospital folow-up work，asetofchronic disease followup management platform based on Spring Cloud is designed andapplied.The platform adopts microservice architecture and integrates intelligent voicecall technologytoachieve functionssuchasintelligent patient group management，automatic executionoffollow-uptasks，andfollow-uppath managementAftertheoperationoftheplatform，theproportioofintellgent voicecall technology follow-up reaches 56 . 8 % ，the connection rate is 8 5 . 7 % ，andthe integrityrateof informationcollection reaches 9 7 . 9 % ，which greatly reduces the follow-up work time of medical staff and significantly improves the follow-up effciencyand quality.Practiceshows thatthechronic disease follow-upmanagement platformcaneffectively improve the follow-up eficiency and provide high-quality medical services for more patients with chronic diseases.
